After solving another incident in Gensokyo and returning to her home—the Hakurei Shrine—Reimu was stricken by a severe flu. Though many people and youkai sent their condolences—many chose not to help, forcing Marisa to become Reimu's caretaker. But before the magician truly begins, Marisa called in some old favors you owe, wanting you to help her. Obligated by honor—along with being a friend—you join Marisa in her endeavor in attending to her friend, walking, and arriving at the Hakurei Shrine.
Calmly sweeping around the living room, you make sure any pieces of trash are relocated onto the shrine's veranda, be it empty packs of ramen, to the elusive dust bunnies. Meanwhile, Marisa is kneeling next to the shrine maiden, placing a cold rag onto Reimu's head to alleviate her fever. Reimu's lower-half is concealed under the soft fabric of the kotatsu table as she lays on the ground, her chest slowly rising with every shallow breath.
After throwing all the trash out the sliding door—with a final small kick of a soda can, you hear Marisa speak up.
"Hey, ${name}, It's been a while since we ate anything. So..." Marisa uses her knee for support as she slowly stands. "I'm going to make some stew. Watch Reimu for me, ok?" Marisa asks with a small smile.
With a moment of thought, you nod a couple of times, setting down your broom. "Sure thing."
Marisa touches the tip of her thumb and index finger, giving you an "OK" hand gesture before walking to the kitchen with a pep in her step.
Briefly taking a few steps, you sit next to Reimu. The shrine maiden slowly looks over to you in a groggy manner; her glazen eyes show you she's not entirely focusing on you. After a moment of silence, Reimu gradually turns to the kotatsu table, mumbling. "Water." she weakly gestures to a small porcelain cup filled with cold water.
